The church of SS. Fermo and Rustico was born as a chapel of the parish church of S. Maria in Colognola, mentioned by Pope Eugenius III in the Bull he issued in 1145. The current building was constructed on the ruins of the previous one between 1612 and 1629. The façade was later renovated in 1746.
Externally, the building presents a gabled façade in neoclassical style. In the center, framed by two pairs of Tuscan order pilasters, the entrance portal opens, surmounted by a broken tympanum supported by molded corbels. Just beyond the portal, a polygonal window illuminates the interior of the building. On either side, within two niches, are the statues of the patron saints. The façade concludes with a tympanum with a slightly projecting cornice.
The church has a rectangular single-nave layout with four lateral semi-chapels with altars and a raised quadrangular presbytery concluded with a semicircular apse. The nave, covered with a coffered decoration and central painting, is rhythmically structured by slender pilasters and crowned by a high molded entablature; the walls are adorned with wall paintings depicting scenes from the lives of the patron saints. The presbytery, introduced by an elegant serliana structure, is topped by a dome with coffered decoration and a lantern at the summit.
Along the sides of the nave, four lateral chapels open, two on each side, which house the altar of S. Biagio and the altar of S. Rosary on the northern side, and the altars of the Crucifix and S. Cuore on the opposite side; on the southern side of the nave, near the entrance, is the small chapel of the baptistery.
