This church was completed in 1695, as confirmed by a plaque above the altar, which also attributes its construction to Marquis Giulio Carlotti.
The church features a portal with the typical shapes of the late 15th century, recovered during its construction, which was likely the entrance to the Palace of the Cavalli, partially demolished to create the current Villa Carlotti. Next to the portal are two recesses containing two capitals, and on the diagonal sides, there are two large octagonal windows.
Inside, the building has a polygonal floor plan. The presbytery is spacious and features a lowered vault; at the center is the altar, supported by two Ionic columns made of red Verona marble and a broken tympanum. An elliptical frame houses the painting of Saint Blaise, with the saint in the center flanked by San Carlo Borromeo and San Francesco, the patron saints of the Carlotti family. The painting, contemporary with the sacred building, is attributed to the Veronese painter Andrea Voltolini. The interior is also rich in frescoes of great historical value.
